Fallout Extreme was an unreleased, team-focused, first- and third-person tactical game designed for the Xbox and PlayStation 2, built on the Unreal Engine by Interplay's 14 Degrees East team. Development began in 2000 for a few months, but the project never secured a stable team or moved beyond the initial concept stage. The title was never formally revealed, and its existence remained a secret until detailed records about it were uncovered and shared in January 2010.
The exact timing of the game's setting remains uncertain. A design document summary states that "nearly a century has passed" since the Great War. The Brotherhood of Steel (in concept art, they resemble the Midwestern branch from *Fallout Tactics*), the "once silent organization," has been growing and taking control of more territory. Their influence now reaches as far as Alaska, where the Brotherhood's new central base is located. Their goals "leave little chance for the survival of radiation or FEV victims." Locals are either forced into the Brotherhood's army as expendable soldiers or turned into slaves, while mutants are systematically eliminated. It is the Brotherhood the player will fight against during the first part of the game. The player leads a group of revolutionaries called The Cause. As the game progresses, this movement gains strength, beginning in Oregon, Washington, and Canada. Every town retaken will pledge its support to the player's cause. But after defeating the Brotherhood, the player will discover the reason behind the Brotherhood's push into Canada and Alaska. The rebels must now cross the Bering Strait, travel through Russia and Mongolia, and eventually reach China to destroy the Doom's Day Missile that could wipe out the remnants of America. The final chapters of the game take place in the Forbidden City, where the Chinese Emperor is in residence. Along the journey, the player will encounter various factions, including the Issaquah Nation, the Inuits, the Montauk, the Horde of Huns, the Siberian Cossacks, and ultimately the Army of the Golden Tiger and the New Imperial Guard.
