P.T. follows an unnamed main character who wakes up in a room with concrete walls and opens a door to a creepy hallway. This hallway endlessly loops and changes its layout. A radio in the room plays a news report about two families brutally killed by their own fathers. Much of P.T.'s story is revealed through scattered clues and the radio's transmissions, though some parts remain open to the player's own theories and interpretations. As the game progresses, strange supernatural events begin—such as the appearance of a ghostly mother from one of the murdered families, the hallway stretching endlessly, and red lights flickering on. The radio also starts picking up eerie transmissions. After the player completes the final puzzle, a cutscene shows the protagonist arriving in Silent Hill, seemingly unaware of his location. He glances behind him before stepping further into the town.
